The Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB LHIN) is responsible for 188 health service providers working together to provide more than 230 programs and services. Our provider groups include: • 56 Community Support Services • 43 Mental Health & Addictions Programs • 17 Supportive Housing Programs • 7 Community Health Centres including 10 sites • 86 Long-Term Care Homes • 9 Hospitals including 23 hospital sites The HNHB LHIN also coordinates the delivery of home and community care services.
